Community Allied Health Care Services

In order to assist our clients to reach their full potential in the community, our allied health care professionals including occupational therapist and physiotherapists amongst others will apply their expertise to holistically assess the client and environment, address their needs, treat and rehabilitate people of all ages.

Community Physio Services

Companionship and Respite Care

One of the most important aspects of 3C Malta will be providing customers with a strong sense of independence and self-esteem. Taking a genuine interest in our client, their life and their environment will help create the true sense of companionship. Whether it is playing a crossword or card game, reading, taking a walk round the block or visiting friends and neighbours, companionship reflects the ethos of 3C Malta. Moreover, through in-house respite services, informal carers will be relieved and given some time out from their carer commitment.

Assistance in Personal Care by Carers

Qualified carers will encourage, enable, and provide assistance in carrying out the ‘activities of daily living’ such as personal hygiene, grooming, hair care, shaving, cleaning teeth/dentures; supervising or assisting with baths/showers and bed baths; assisting with dressing/undressing; assisting with mobilization activities including turning, transferring, exercising, and walking; assisting with toilet and/or commode needs; assisting with eating or feeding and prompting the client to take their prescription medications. Some of these services may be provided at home or any other community care setting.

Domestic Care

Our community carer will provide assistance with domestic tasks or instrumental activities of daily living. Tasks may include dusting, washing flooring, vacuuming carpets and furniture, changing bed linen, washing, drying, ironing and folding laundry; caring for house plants; taking out garbage and general tidying of living spaces as required or requested. It also includes caring for pets as well as grocery shopping, preparing menus and meals, cooking, baking, serving, assisting with eating, washing and drying dishes and general tidying up.

Transportation

Our community carer may drive or escort the client to hospital appointments or errands and provide a helping hand or a shoulder to lean on. Our carers will take note of the appointments and can handover to the client’s respective informal carers.

Nursing Care

We can provide safe, effective and evidence- based nursing care and the support you need to age at home, recover from an acute episode of illness or post-surgery as well as provide the needed information and support to facilitate management of chronic diseases or illnesses. Whether being diagnosed with a new condition, syndrome or disease or suffering from a chronic condition, our professional nurses will provide you with evidence-based knowledge and education and guide you on your next steps to follow. Various educational programmes and information sessions can be organised to help you understand more your condition and give you professional tips on how to manage your condition or control its effects

Professional Nurses will carry out skilled nursing activities directly related to your care through assessing, planning, implementing and evaluation. Following a thorough assessment, clients who have complex needs and require assistance in carrying out their ‘Activities of Daily living’ will be assisted by nursing staff in line with the agreed upon care plan.

Our Professional and experienced community nurses can provide various professional services, such as:

Parameter Monitoring

Monitoring of Vital signs, blood glucose, blood pressure, pulse and respiration monitoring as well as other specific monitoring required by our clients, such as; Oxygen blood saturation monitoring and management of Non-Invasive ventilation or Positive End Expiratory Pressure (PEEP) masks.

Medication Management

Preparation and /or administration of Oral, Ocular, Nasal, Rectal, Otic, Inhalation, Cutaneous (Topical) or Transdermal medications. Adequate information and education will be also provided to clients and their informal carers to ensure better understanding and compliance to prescribed medication. Our professional experienced nurses will prepare and administer medications via intra-muscular and Subcutaneous injections as prescribed by medical professionals.

Catheter Care and Management

Management and care of various indwelling catheters – be it Peritoneal, Urinary, suprapubic, Port-a-cath, Vascath or any other catheters in situ.

Urinary and Suprapubic Catheter management is carried out by a professional registered experienced nurse, including the Insertion, removal and general care of the catheter in situ. Professional support, advice and education is provided in order to promote compliance to the client’s own care plan.

Care of the Diabetic Client

Administration of insulin as prescribed in the client’s treatment regime and monitoring of blood glucose as needed. Education and training on the administration of the medication itself and the importance of diet, exercise and foot care amongst others will also be provided so as to equip our clients with the necessary tools to be autonomous, make informed decisions and be active participants in their care.

Specific Post-Operative Rehabilitation Programs

Following a holistic assessment and in collaboration with the client and informal carers, a rehabilitation program and care plan will be formulated in line with the consultants/ surgeons instructions and based on the latest evidence based practice and empirical research.

Feeding

A qualified health care professional specialising in Diet, hydration and nutrition will be provided to guide the clients in their nutritional difficulties and their respective weight management and maintenance. Specialised professional nursing care will be provided for patients needing care of Naso-Gastric feeding (NGT) or Percutaneous Endoscopic Gastrostomy (PEG). The professional nurse will be able to assist in the insertion of prescribed NGT and their monitoring as well as provide all the necessary support and advice.

Wound Management

Management and care of acute, surgical as well as chronic wounds and/or ulcers through comprehensive wound assessment and change of dressing/s according to the specific requirements of the wound. Removal of sutures can be done according to the Consultant’s/ Surgeon’s instructions.

Physiotherapy Service

Whether you are recovering from an injury or need help managing an ongoing medical condition, we can work with you to increase your strength, balance and mobility. At 3CMalta our community physiotherapists will start with an assessment of your loved ones’ needs, requirements and capabilities in the comfort of their own home, taking into consideration environmental factors to reduce the risk of falls and accidents amongst others. Our physiotherapists will visit you in your home to:

  • Assess your strength, balance and mobility

  • Discuss your Goals

  • Provide falls assessments, prevention, education and training

  • Get an understanding of your daily activities

Occupational Therapy

At 3CMalta, our occupational therapists can help you and your loved ones to continue with a lifestyle and everyday activities that are meaningful and purposeful, while maintaining your independence. Amongst other things, our services include enablement and prevention of functional deterioration through individualised programs. Our OT will visit you or your loved ones in the comfort of your home to assess current abilities and together create a car plan aimed at teaching new techniques, provide advice on equipment and make you home safer.

Speech Therapy

There are a number of situations where a client could benefit from speech therapy. At 3CMalta our Speech therapist can help someone to recover from a stroke, or it may be beneficial in situations where you have noticed your loved one is having difficulties responding to or answering questions. Other reasons could be difficulty in swallowing, weakened vocal cords amongst other advantages.
