- A side table for water, medication, and tissues
- Good lighting
- A bell or phone within reach
2. Confirm the Medication Plan
Double-check:
- What medications need to be taken, and when
- Any changes made while in hospital
- If a nurse or carer will help with administration
Keep a printed list nearby — this helps avoid confusion or missed doses.
3. Know Who to Call (And When)
Have emergency numbers and contact info for your GP, pharmacy, and healthcare provider clearly written and placed somewhere visible — like on the fridge.
4. Stick to Follow-Up Appointment
Many people skip post-hospital checkups because they “feel fine” — but these appointments are essential. Whether it’s a wound check, physiotherapy, or lab results, don’t delay.
5. Watch for Warning Signs
Keep an eye out for symptoms like:
