Scope of work: The Post holder shall provide comprehensive nursing care in the community setting that assists clients to achieve optimum health and quality of life in situations of chronic disease, acute illness, injury or through the process of dying. S/he shall practice independently at all times across one’s professional discretion and decisions, in line with the Code of Practice for qualified nurses in Malta, the Laws of Malta. Community nurses working with 3C Malta are expected to apply the nursing process to ensure the provision of good quality nursing care through holistic assessment of needs, the development, implementation and evaluation of nursing care plan/s, the education of clients and informal carers in collaboration with all related stakeholders.
Place of employment: Client homes, work sites outside of client home and other community settings as required by 3C Malta management.
Work hours: This position may require various shifts at various times (including nights, weekends, and sleep-over) in one or more work settings.
Main Professional Responsibilities
Community nurses working with 3C Malta should competently and independently:
- Enforce and implement 3C Malta’s Vision, Mission and Core Values in his/her own work with enthusiasm and commitment;
- Demonstrate good communication skills with colleagues, clients and informal carers in a professional manner.
- Develop and maintaining respectful, cooperative working relationships to contribute to the integrated, seamless delivery of services in the community.
- Demonstrate nursing presence and compassion by providing emotional, psychological, and spiritual support to clients and informal carers.
- Establish therapeutic relationships with clients and their informal carers, utilising an empathic approach.
- Carry out an effective holistic needs’ assessment (bio-psycho-social spiritual) of potential service users.
- Ensure a joined goal setting and planning of care in collaboration with the clients and their informal carers.
- Be able to plan and organise own workload ensuring that the interests of the clients are met at all times.
- Promote 3C Malta clients’ independence by establishing care goals; teaching clients and their informal carers to understand condition, medications, and self-care skills and address any concerns that arise.
- Utilizes adult learning principles and established teaching materials to provide health teaching to clients and families.
- Collaborate with various members of the interdisciplinary team in order to provide a high-quality holistic patient care.
- Demonstrate respect and sensitivity for diversity in beliefs, values and cultural practices of all staff, clients and informal carers.
- Support and enable self-care /independence when possible and self-management in long term conditions.
- Under the direction of the 3C Malta Management carry out a range of complex technical nursing skills independently according to the needs of the specific service user in a confident, competent and proficient manner in accordance with the orders of a physician and under a plan of care.
- Provide individuals with the opportunity to achieve significant and measurable progress in health needs.
- Be able to evaluate the nursing care implemented in view of determining whether the expected outcome/s has been achieved and to adjust the plan of care accordingly in an independent manner.
- To refer to appropriate entities and professionals when needed.
- Complete and maintain the appropriate documentation as stipulated by the agency’s standards and policies and provides and requests handover from colleagues and other health care professionals to ensure continuity of care.
- Maintain privacy and confidentiality at all times in accordance with the Malta Nursing Code of Ethics as well as GDPR guidelines.
- Conduct risk assessments and promote a safe working environment for team members including carers.
- Mentor junior or new staff as well as students who are on placement with 3C Malta.
- Maintain and improve personal and professional knowledge, competence and development and fulfil continuing education requirements as may be required.
- Protect clients and employees by adhering to safety initiatives such as infection-control policies and protocols, medication administration and storage procedures and take prompt and necessary action when there are incidents, such as patient falls, medication errors and/or near misses.
- Independently initiate life preserving immediate measures and to carry out measures in crisis and emergency situations.
Qualifications/Requirements
- Be in possession of a certificate of registration as a First Level Nurse with the Council for Nurses and Midwives (Malta).
- Minimum five (5) years’ experience as a first level nurse.
- Compassionate, patient and caring nature.
- Must possess a valid certification in Basic First Aid and AED.
- Demonstrated sound clinical nursing knowledge and experience in providing direct client care.
- Proven ability to apply clinical nursing skills in a home care setting.
- Proven ability to problem solve and to adapt nursing interventions.
- Valid driver’s licence with a reliable method of transportation.
- In possession of clean criminal background and able to provide a police conduct on engagement and have a full vaccine screen.
- Fluent in Maltese and English Language.
- Proficient in IT and communication skills.
- Excellent interpersonal skills.
- The ability to work independently and resourcefully.
- Maintain a professional attitude and demeanour, including courteous interactions with team members, supervisors and others encountered in the course of work.
- An ability to use own judgement, resourcefulness, common sense and local knowledge, to respond to patient’s enquiries and requests.
